Oklahoma City, OK – NTSB: Pilot in fatal crash was under investigation

OKLAHOMA CITY (AP) The National Transportation Safety says the pilot of a helicopter that crashed near Tahlequah and killed both people on board was under investigation.

The NTSB's final report on the Oct. 14 crash that killed pilot Mark Mahaney and passenger Darci Scraper says Mahaney was being investigated by the Federal Aviation Administration. The report says Mahaney lied on his medical certificate application and that he failed to report to the FAA three alcohol-related incidents on his driving record.

The FAA said Mahaney had not responded to a letter telling him of the investigation.

The state Medical Examiner's Office has said an autopsy found Mahaney had methamphetamine and painkillers in his system.

The report does not list a probable cause of the crash.
